Permanent Bookings It is assumed that permanent bookings will operate for the whole year. Cancellation of Care: A two weeks’ notice will need to be given to cancel any of your child/ren’s care. To cancel care, we ask that you complete a Cancellation of Booking form which you can collect at the centre (located in the red tray, labelled ‘Cancellation of Booking Forms” where you sign in/out), or download from the SMOOSH website. Once the form has been completed you will need to submit the form to the Director (or a Staff member to forward on to the Director) or if preferred to email you can email it to; smoosh.stmonicas@cg.catholic.edu.au. Please note: Your two weeks’ will take effect from the date that we receive it. For more information regarding this please see the Centre Director. |
Casual Bookings All casual bookings must be phoned through to the Director by 12:00pm the day of care. Parents will be advised via phone or email by 2:00pm if a casual place is available. A $5.00 penalty fee will also be charged for failure to notify SMOOSH of a student’s absences/attendance. NOTE: A causal booking may include multiple days and/or multiple children. |
Vacation Care Program Bookings The Vacation Care outline (including planned excursions) and Vacation Care program booking form is provided published in the following section. Parents are required to return the form (either by email or in person) by Wednesday of Week 9. Permission to attend excursions must be provided through the submission of individual permission notes with the Vacation Care booking request form. |
Medications: Medications will only be given under the following conditions: • The service medication form is completed and signed by Parent/Guardian, including all relevant details of the medication to be administered. • All medications must be in its original container with a pharmaceutical label clearly showing: Child’s Name Name of Medication Specific Administration Instructions Dispensary and Expiry Date Non-prescription medications, including herbal preparations, will only be administered if a Medical Practioner has provided written consent for its administration to the child. Under no circumstances is any type of medication to be left in children’s bags. Please hand all medications to a staff member and complete the medication form. |
